But with Nar games, the disease isn't quite defined and with another perception, doctors (PC's) will react different.

Nar isn't "the disease isn't defined." Nar is "it's gangrenous, do you amputate or try to save the limb?," or "it's cancer, radiation or chemotherapy?" What's undefined is the solution, and no option should be easy.
