Because penalty is often mechanically reinforced and immediate, while most reward systems are arbitrary and appear only at the end of a session. As Vincent said- bad design.

yes, yes! I love this type of play! I'm working to codify it so injury/penalty you have to deal with NOW and reward is something to have to work for and manage resources-like. :)
